What is the Macclesfield
Ageing Well Clinic?
The Ageing Well clinic has been set up in conjunction with your GP to help support you with any medical needs you may have. It has been designed as a “one-stop” clinic made up of a multidisciplinary team, providing personal consultations, as well as an opportunity to meet other people from the community in a similar situation to you.
Why have I been invited?
Your GP has referred you to the clinic as they feel it will meet a number of your needs. You will have access to talk with a variety of healthcare specialists, who will be able to provide information and one-to-one support in a relaxed and confidential setting.
When and where is the clinic held?
You will receive a letter to advise the date and time of your clinic appointment. The clinic is held once a month on the 1st Floor of Waters Green Medical Centre. On arrival, please report to reception.
Please allow 2 hours in total for your appointment.


What will happen at the clinic?
A confidential consultation room will be available for you to meet with different members of our multidisciplinary team. You will have be given an appointment with each of the following:
- A pharmacist from the medicines optimisation team who will look at your current medications with you
- A senior nurse and physiotherapist or occupational therapist to look at your general health and wellbeing
- A Wellbeing Co-ordinator to help with access to support and services in the local community
Light refreshments will be available.

We will also be in touch 2 weeks before the clinic to check if there is anything we can help with.
What happens next?
If you require a referral into another service (or services) following the clinic, this will be arranged for you. A summary of your appointment will be sent to your GP, and you will receive a follow up call from a member of the team to see how you are getting on afterwards.
You will be asked for your feedback on the clinic before you leave, but if you do have any comments afterwards please don’t hesitate to contact us. We would love to hear any comments or suggestions you have as these will help us in shaping our service in the future.
What will I get out of attending the clinic?
The clinic has been developed to provide proactive care and support, allowing you to see the specialists you need in one convenient appointment. Our aim is to create a personalised healthy-ageing care plan encompassing what matters most to you.
